Paperwork: Save time by printing and filling out new patient forms before you get here.
Running on time: We know your schedule is busy and that your time is valuable. If you arrive more that 15 minutes late for your appointment, you will need to reschedule your appointment.
Cancellations: Please call within 24 hours if you are unable to keep your scheduled appointment, or a $50 fee will be applied to your account.
